You may be recovering from a joint replacement, stroke, head injury, amputation, or other surgery. You may need treatment to manage your pain or extra care for a wound. In any case, the care team at Cabinet Peaks Medical Center is here to help you get back to the business of being well!
What is Swing Bed?
Swing Bed offers inpatient care to those needing extra time to heal or strengthen before returning home. The term swing bed came from the idea that the patient will "swing" or transition, from inpatient acute care through the program to return home. Swing Bed allows providers to swing the level of care from acute to skilled rehabilitation.
When is Swing Bed appropriate?
Swing Bed may be considered for patients who have had surgeries, a lengthy illness, or an accident. A patient can transfer from another facility to Cabinet Peaks Swing Bed Program to be closer to family and friends or, if the patient is already in Cabinet Peaks Medical Center, simply change patient status to receive Swing Bed care. Cabinet Peaks Swing Bed Program's top priority is to assist patients in regaining their independence and mobility through intensive therapeutic sessions.
Generally, to qualify for swing bed the patient must:
have at least a three consecutive day stay as an acute patient within the last 30 days.
be certified by a provider as needing skilled services.
require daily skilled services which cannot be provided in a lesser care setting.
Swing Bed Referral Process:
Referrals to Swing Bed come from the attending provider once it has been determined that the patient no longer requires an acute care level of service, but will still have a skilled reason to stay in the hospital setting.
The Cabinet Peaks Swing Bed Case Manager reviews the patient's medical record and confers with the provider, therapy staff, and the patient's nurses to determine the skill need.
For out-of-house referrals, the admitting provider and the Cabinet Peaks Swing Bed Case Manager work together to make a decision about the patient's Swing Bed level based on information from the referring facility.
What services are provided?
Hospitalist
All of our treatment plans are developed under the supervision of a physician.
Nursing Services
Provides wound care, daily intravenous medications,intramuscular injections, subcutaneous injections, and tube feedings.
Respiratory Therapy
Provides pulmonary disease education and treatment, oxygen needs, cardiac monitoring and support, and the administration of hand-held nebulizers and chest physiotherapy. Our Respiratory Therapy team can also coordinate a seamless transition to Cardiac or Pulmonary Rehabilitation and home oxygen needs.
Swing Bed Case Manager
Assists with discharge planning, personal resources, financial needs, arrangements for home health services, and any medical equipment.
Nutritional Services
Offers medical nutrition therapy, education, and customized meal plans to meet dietary needs and personal preferences as optimal nutrition is important in recovery.
Rehabilitative Services
Our skilled rehabilitative services team evaluates each patient's capacity and develops a program to meet achievable goals. Rehabilitative services are provided a minimum of 5 days a week. For certain conditions, such as total joint replacements, physical therapy treatments are provided up to two times a day and 7 days a week.

Payment
The Swing Bed Program is geared toward those with Medicare benefits, as this is a Medicare based service. However, we can accommodate people of all ages. Some private insurance companies will negotiate this service and arrangements can be made for patients with other payer sources such as Medicaid.
Medicare will cover the first 20 days of Swing Bed stay at 100%, as long as there is a skilled service needed and provided.
